About The Position

Agape Care Group is seeking a part-time Social Worker (MSW) to join their team. This role is for a hospice medical social worker who is passionate about helping patients and their families receive compassionate care during their end-of-life journey. Responsibilities include psychosocial evaluations and ongoing counseling for patients and families, providing emotional support in accordance with the plan of care. Team members have access to supportive leadership and professional development opportunities for advancement. Requirements

A heart to serve patients and families and a passion for providing the best possible care
MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
Current state license as a social worker
2+ years of clinical work experience, preferably in healthcare or hospice
Reliable transportation
Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively

Responsibilities
