About The Position

This clinical role is part of the expanding Pediatric IBH team, working alongside pediatric and primary care offices as a member of the primary care team. The position helps address anxiety, depression, and behavioral concerns in children and adolescents through evidence-based behavioral health interventions. The role involves clinical triage, assessment, and evaluation, with support from clinical supervision hours. We are seeking candidates with strong problem-solving skills, excellent communication, and a commitment to delivering exceptional care to patients and families. This will be a hybrid position, with at least 2 days onsite per week at one of our clinics in Lafayette, Muncie, Indianapolis, or Bloomington.

Requirements

  • Master's Degree in Social Work required.
  • Permanent Indiana license as an LSW required.
  • Crisis Prevention Intervention (CPI) or alternative de-escalation training required within 6 months of hire.
  • 0-3 years relevant healthcare, mental health, or community experience preferred.
  • Evidence-based psychosocial assessment, short-term therapeutic counseling, and crisis intervention skills
  • Knowledge of disease processes and human behavior related to social needs
  • Complex discharge planning expertise for safe patient transfer
  • Legal knowledge of child and adult protective mandates (state and county)
  • Principles of growth, development, and end-of-life care
  • Ability to identify decision-makers and lead goals of care conversations
  • Experience adapting services for diverse populations across geographic, cultural, and financial backgrounds
  • Computer literacy and proficiency in electronic medical records (EMR)
  • Building rapport and establishing trust with patients and families
  • Providing compassionate, solution-focused support
  • Effective communication across multidisciplinary teams
  • Organizational skills for planning and discharge coordination
  • Maintaining confidentiality and HIPAA compliance

Responsibilities

  • Meet face-to-face with patients and families, providing assessments, counseling, and resource coordination
  • Collaborate with healthcare teams to ensure safe, timely, and effective patient care and discharge planning
  • Interpret and communicate complex psychosocial needs, modeling caring practices to other caregivers
  • Assess psychosocial risk factors through evaluation of prior functioning, support systems, reactions to illness, and coping skills
  • Counsel patients and families regarding emotional, social, and financial impacts of health conditions or disabilities
  • Access and mobilize family and community resources to meet identified needs
  • Conduct mental health assessments upon physician referral
  • Serve as a key point of contact for cases involving child abuse/neglect, elder abuse/neglect, institutional abuse, and domestic violence
  • Act as a liaison between IU Health and local protective services (APS/DCS)
  • Provide crisis intervention, de-escalation, and support during acute situations, including potentially dangerous scenarios
  • Support complex discharge planning and care transitions
  • Provide specialized mental health services, including crisis and addiction assessments, with recommendations for psychiatric or community resources
  • Lead interventions at end-of-life, guardianship, foster care, adoption, and other complex cases
