Social Worker – MSW / LSW Location: University Hospital, 550 University Blvd, Suite 2180, Indianapolis, IN 46203 Department: Integrated Care Management - Social Work Schedule: Monday – Friday, 8:00 AM – 4:30 PM Position Overview: The Social Worker provides face-to-face assessments and counseling with patients and families, collaborating with the healthcare team to deliver safe, timely, and resourceful care. This role involves interpreting complex psychosocial needs, providing crisis intervention, facilitating discharge planning, and acting as a liaison with protective services and community agencies. This position is part of the Specialty Care Social Work team working with patients across multiple clinics, including Endocrinology, Rheumatology, Pulmonary, Renal/Nephrology (pre/post kidney transplant and dialysis), Infectious Disease (HIV/AIDS), and Interventional Radiology. Key Responsibilities: Meet face-to-face with patients and families to assess psychosocial needs and provide counseling. Communicate and collaborate with all members of the healthcare team to coordinate care effectively. Interpret and communicate complex patient and family needs; model caring and professional practices to other caregivers. Assess psychosocial risk factors via evaluation of prior functioning, support systems, reactions to illness, and coping abilities. Counsel patients and families regarding emotional, social, and financial impacts of illness/disability. Access and mobilize family and community resources to meet patient needs. Provide mental health assessments upon physician referral. Deliver therapeutic interventions to support complex discharge planning. Serve as the primary contact for cases involving child abuse/neglect, elder/elderly abuse/neglect, institutional abuse, and domestic violence. Act as liaison between Indiana University Health and local child and adult protective teams (APS/DCS). Provide intervention in guardianship (temporary/permanent), foster care, adoption, and mental health Level II placements. Offer specialized mental health services, including crisis intervention, addiction assessments, and community resource recommendations. Support individuals in dangerous or potentially dangerous situations, taking a leadership role in clinical intervention. Utilize de-escalation techniques to support individuals in acute crises and prevent violence or episodes of physical aggression. Conduct effective interviews with victims, perpetrators, family members, and other stakeholders. Provide intervention for end-of-life, guardianship, foster care, and adoption cases. Qualifications & Requirements: Education: Master’s Degree in Social Work from an accredited college or university. Licensure: Permanent Indiana LSW licensure required. Certifications: Crisis Prevention Intervention (CPI) or an alternative de-escalation training required within 6 months of hire. Experience: Relevant healthcare, mental health, or community-based experience preferred. Knowledge & Skills: Evidence-based psychosocial assessments, short-term therapeutic counseling, and crisis intervention for mental health and substance use disorders. Knowledge of disease processes and human behavior related to health and social needs. Complex discharge planning skills for safe patient transitions. Understanding of state and county adult and child protective legal mandates. Principles of growth and development across the lifespan, including end-of-life care. Ability to identify decision-makers and facilitate goals-of-care conversations. Experience in adapting services to diverse geographic, cultural, and socioeconomic populations. Ability to develop and maintain strong relationships with hospital administrators, medical teams, law enforcement, APS/DCS, schools, probation, and community agencies. Computer literacy and proficiency in documentation within an electronic medical record (EMR). Core Competencies: Strong communication and active listening skills.
