About The Position

Social Worker – MSW / LSW Location: Arnett Hospital, Lafayette Department: Integrated Care Management Schedule: Monday – Friday, 8:00 AM – 4:30 PM Note: Likely 1 on-call holiday per year Schedule may include on-call and holiday rotations. Position Overview: The Social Worker conducts face-to-face assessments and counseling with patients and families, collaborating with the healthcare team to ensure safe, timely, and resourceful care. The role involves interpreting complex psychosocial needs, providing crisis intervention, supporting discharge planning, and serving as a key liaison with protective services and community agencies.

Requirements

  • Master’s Degree in Social Work from an accredited college or university.
  • Permanent Indiana LSW licensure required.
  • Crisis Prevention Intervention (CPI) or an alternative de-escalation training within 6 months of hire.
  • Evidence-based psychosocial assessment, short-term therapeutic counseling, and crisis intervention for mental health and substance use disorders.
  • Understanding of disease processes and human behavior related to health-related social needs.
  • Complex discharge planning skills for safe patient transitions.
  • Legal knowledge of state and county adult and child protective mandates.
  • Principles of growth, development, and end-of-life care.
  • Ability to identify decision-makers and facilitate goals-of-care conversations.
  • Experience in adapting services to diverse geographic, cultural, and financial populations.
  • Strong relationships with hospital leadership, medical teams, law enforcement, APS/DCS, schools, probation, and community agencies.
  • Computer literacy and proficiency in documentation within an electronic medical record (EMR).
  • Ability to respond effectively to violent or potentially violent situations using de-escalation techniques.
  • Excellent communication and active listening skills.
  • Strong crisis intervention, de-escalation, and violence prevention skills.
  • Ability to work independently and within a multidisciplinary team.
  • Confidentiality and HIPAA compliance.
  • Cultural sensitivity and professionalism.

Responsibilities

  • Meet face-to-face with patients and families to assess psychosocial needs and provide counseling.
  • Collaborate with the healthcare team to facilitate a coordinated, safe, and timely episode of care.
  • Interpret and communicate complex patient and family needs while modeling caring and professional practices.
  • Assess psychosocial risk factors through evaluation of prior functioning, support systems, reactions to illness, and coping skills.
  • Counsel patients and families regarding emotional, social, and financial impacts of illness/disability.
  • Access and mobilize family and community resources to meet identified needs.
  • Provide mental health assessments upon physician referral.
  • Deliver therapeutic interventions to support complex discharge planning.
  • Serve as the primary contact for cases involving child abuse/neglect, elder/elderly abuse/neglect, institutional abuse, and domestic violence.
  • Act as liaison between Indiana University Health and local child and adult protective teams (APS/DCS).
  • Provide intervention in guardianship (temporary/permanent), foster care, adoption, and mental health Level II placements.
  • Offer specialized mental health services, including crisis intervention, addiction assessments, and community resource recommendations.
  • Support individuals in dangerous or potentially dangerous situations, providing leadership in clinical interventions.
  • Use de-escalation techniques to support patients in acute crises and prevent violence.
  • Conduct effective interviews with victims, perpetrators, family members, and key stakeholders.
  • Provide intervention for end-of-life, guardianship, foster care, and adoption cases.
