Requirements

  • Strong interpersonal and counseling skills.
  • Ability to navigate complex social systems and advocate for patients.
  • Commitment to ethical practice and continuous learning.
  • Master of Social Work (MSW) degree from a CSWE-accredited School of Social Work (required).
  • Current, active Social Work License OR successful completion of the social work licensing exam within 9 months of employment.
  • Acceptable licenses include:
  • Clinical Social Worker (CSW)
  • Licensed Social Worker (LSW)

Nice To Haves

  • Previous healthcare experience preferred.

Responsibilities

  • Conducting comprehensive psychosocial assessments and providing crisis intervention, counseling, and discharge planning.
  • Connecting patients and families to community resources, federal/state programs, and support systems.
  • Utilizing therapeutic methods such as brief goal-directed therapy, crisis intervention, and psychosocial support.
  • Advocating for patients regarding insurance coverage and post-acute care services.
  • Collaborating with healthcare teams to create individualized care plans and educate families on medical decisions, advance directives, and end-of-life care.
  • Maintaining accurate documentation and adhering to professional ethics and confidentiality standards.
