Requirements
- Strong interpersonal and counseling skills.
- Ability to navigate complex social systems and advocate for patients.
- Commitment to ethical practice and continuous learning.
- Master of Social Work (MSW) degree from a CSWE-accredited School of Social Work (required).
- Current, active Social Work License OR successful completion of the social work licensing exam within 9 months of employment.
- Acceptable licenses include:
- Clinical Social Worker (CSW)
- Licensed Social Worker (LSW)
Nice To Haves
- Previous healthcare experience preferred.
Responsibilities
- Conducting comprehensive psychosocial assessments and providing crisis intervention, counseling, and discharge planning.
- Connecting patients and families to community resources, federal/state programs, and support systems.
- Utilizing therapeutic methods such as brief goal-directed therapy, crisis intervention, and psychosocial support.
- Advocating for patients regarding insurance coverage and post-acute care services.
- Collaborating with healthcare teams to create individualized care plans and educate families on medical decisions, advance directives, and end-of-life care.
- Maintaining accurate documentation and adhering to professional ethics and confidentiality standards.
