Position Summary: A Social Worker (LSW) assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems. Collaborates with various medical providers and clinical teams to provide patient and family advocacy, navigation through cancer care processes, assistant with grants and all other barriers they face during their oncology care. Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives. Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions. Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan. Provides individual, family and group therapy. What We Are Looking For: Bachelors required; Masters preferred Board Certified Social Worker, or related field (i.e. counseling) in the state of Kentucky (CSW, LPCA, LCSW) Board eligible within 6 months of hire Handle With Care within 30 days of hire. Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
