About The Position

We are seeking a compassionate MSW / LSW to lead and support group services through our Moms Heal Together Clinic, a community health program dedicated to supporting pregnant and postpartum individuals (up to 2 years postpartum) impacted by substance use—presently or in their past. This grant-funded position is based in Bloomington, with support for expansion into Orange and Greene counties, requiring light travel once weekly. This role is ideal for those seeking to return or shift in their career, leveraging life experience and a passion for supporting individuals facing substance use challenges. The position involves direct, face-to-face engagement with patients, providing psychosocial assessments, counseling, discharge planning, crisis intervention, and community resource mobilization. Note: This position is funded through Safety Pin a Grant Program. The funding is scheduled for renewal, and ongoing employment is contingent upon continued funding support. While the current grant supports the position for the next two years, future funding renewal is possible, and employment may continue based on funding availability.

Requirements

  • Master’s Degree in Social Work from an accredited institution.
  • Permanent Indiana LSW licensure required.
  • 0-3 years in healthcare, mental health, addiction, or community services preferred.
  • Certification in Crisis Prevention Intervention or similar de-escalation training required within 6 months of hire.
  • BLS certification may be required.
  • Evidence-based psychosocial assessment, short-term therapeutic counseling, and crisis intervention.
  • Knowledge of the disease process, human behavior, and health-related social needs.
  • Complex discharge planning skills.
  • Legal knowledge of child and adult protective services mandates.
  • Ability to adapt services to diverse populations geographically, culturally, and financially.
  • Strong interpersonal and collaboration skills.
  • Computer literacy and proficiency in electronic medical records.

Responsibilities

  • Meet face-to-face with patients, assessing psychosocial needs and providing counseling.
  • Collaborate with healthcare teams to ensure safe, timely, and resourceful patient care.
  • Interpret and communicate complex patient needs; role-model caring practices.
  • Assess psychosocial, developmental, and cultural needs, including prior functioning, support systems, and coping mechanisms.
  • Counsel individuals regarding emotional, social, and financial impacts of substance use and related health issues.
  • Access and mobilize community and family resources to meet individual needs.
  • Conduct mental health assessments upon physician referral.
  • Facilitate complex discharge planning and therapeutic interventions.
  • Serve as the primary point person for cases of child abuse/neglect, elder abuse/neglect, institutional abuse, and domestic violence; act as liaison with protective agencies.
  • Provide crisis intervention and de-escalation support during dangerous or potentially violent situations.
  • Support guardianship, foster care, adoption, and mental health placements.
  • Participate in training and supervision activities, supporting best practices in mental health care.
  • Lead and support group services tailored to postpartum and substance use recovery.
