Position Summary The Licensed Social Worker (LSW) is responsible for providing comprehensive psychosocial support services to residents and their families. This role works collaboratively with nursing, therapy, medical staff, and administration to ensure residents’ emotional, social, and discharge planning needs are met. Why Work with Us? Requirements

  • Current Licensed Social Worker (LSW) license in the State of Maine
  • Bachelor’s degree in social work or related field required
  • Experience in long-term care, healthcare, or geriatric settings preferred
  • Strong communication, organizational, and advocacy skills
  • Ability to work effectively as part of an interdisciplinary team
  • Ability to effectively understand and communicate verbally and in writing, in English, is required.
  • Basic computer skills

Responsibilities

  • Conduct psychosocial assessments upon admission and as needed
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Provide counseling and emotional support to residents and families
  • Assist discharge planning
  • Connect residents and families with community resources and benefits
  • Support residents during transitions, changes in condition, or end-of-life care
  • Maintain accurate and timely documentation in compliance with state and federal regulations
