How you’ll make an impact in this role Design comprehensive, personalized care pathways that bridge the gap between hospital and home, ensuring every patient’s values and family preferences are at the heart of their recovery. Champion patient needs by identifying and removing systemic barriers, ensuring every individual has the resources and advocacy required for a seamless transition. Orchestrate clinical outcomes by maintaining a continuous, high-level dialogue with the multidisciplinary team to ensure care plans are executed flawlessly. Safeguard the integrity of the patient journey by overseeing clinical documentation and evaluating service appropriateness to guarantee the highest standard of post-discharge care.

Requirements

  • Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
  • Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Nice To Haves

  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
