The Social Services Specialist for Healthy Families responsibilities include: Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community The Social Service Specialist II for Healthy Families provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning for Mom/Baby They are knowledgeable about treatment, social and economic implications for families as it pertains to their age, stage of development and discharge needs Possess a clear understanding of discharge and transition planning and linkage with community resources Demonstrates skills in planning, organizing, and managing multiple functions and complex processes Position details: Primary Location: Morton Plant Hospital - Clearwater, FL Secondary Location: Mease Countryside Hospital - Safety Harbor, FL(occasional) Status: Part time - 40 hours per pay period Schedule: Every Friday and Saturday 8:00 AM - 4:30 PM                      Every other Wednesday 8:00 AM - 4:30 PM On Call: No 

Requirements

  • Masters in Social Work, Sociology, or Related Field
  • 6 months Internship or
  • 1 year Social Work in an acute care or hospital setting

Nice To Haves

  • ACM (Case Management) or
  • CCM (Case Manager)
  • Masters Degree in Social Work
  • Social Work experience with Mom/Baby and/or children

Responsibilities

  • Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community
