About The Position

Sinai Residences is seeking a compassionate and experienced Social Services Coordinator to join our team.  This role supports residents and families by guiding transitions across the continuum of care and promoting overall health, wellness, and quality of life within our community. The position offers an opportunity to make a meaningful impact through effective relationship management and program oversight within a supportive organizational environment.

Requirements

  • Bachelor’s degree or higher in Social Work or a related field required
  • Experience in senior living, healthcare, or resident services preferred
  • Strong communication, problem-solving, and interpersonal skills

Responsibilities

  • Assess residents’ social, wellness, and healthcare needs
  • Support residents and families during transitions across levels of care
  • Collaborate with interdisciplinary teams and attend care coordination meetings
  • Follow up with residents after hospitalization or significant health events
  • Maintain accurate and confidential documentation
  • Promote internal services and wellness programs
  • Develop and maintain relationships with internal and external resources
