About The Position

The Mather is a luxury senior living community, and we are seeking a compassionate, organized, and mission‑driven Social Services Coordinator to join our team in a full‑time capacity, supporting The Life Centre, our boutique care environment offering skilled nursing, assisted living, and memory support. This role is ideal for a social services professional with experience in long‑term care, hospital, or related healthcare settings—or a four-year degree in social services or a related field—who brings strong communication skills, a thoughtful approach to customer service, and the ability to manage multiple priorities with care and confidence. You will serve as a trusted advocate and partner to residents and families, supporting assessments, admissions, care planning, transitions, and discharge processes while navigating state and federal regulations with professionalism and compassion. The ability to intervene effectively in crisis situations, collaborate within an interdisciplinary team, and maintain required certifications such as CPR/AED is essential, and candidates with an MSW, LSW, or LCSW are especially encouraged to apply. The schedule for this full‑time, hourly position follows a rotating pattern: Week 1 — Monday through Friday; Week 2 — Tuesday through Saturday, and requires scheduling flexibility to best support residents, families, and care teams.

Requirements

  • 2+ years of long term care, hospital or related healthcare experience in leadership role; or 4 year college degree in social services or related field.
  • Excellent verbal and written skills.
  • Ability to multi-task and intervene in crisis/conflict situations.
  • Excellent customer service techniques and approach.
  • Knowledge of related state and federal regulations.
  • CPR/AED certification.

Responsibilities

  • Coordinates and documents; residents Plan of Care and Care Plan Conferences, completes the social services portion of the MDS, social services provided to residents, and family members or responsible parties.
  • Facilitates and maintains documentation of Advance Directive and Resident Rights in compliance with company policies, federal and state regulations.
  • Plans and coordinates transitional movement, discharge and post-discharge care needs.
  • Facilitates admissions process.
  • Serves as resident advocate to assist residents, resident representatives, or resident groups and responds in writing to any complaint on behalf of residents.
  • Actively participates in QAPI process.
  • Coordinates and/or provides education and support to staff, residents and families pertaining to psych-social issues including illness, aging, trauma informed care, loss, and death and dying.
  • Participates in and supports community outreach events.
  • Supports budget process and maintains fiscal accountability.
  • Work in conjunction with the care venue team to support compliance with Medicare guidelines including the administration of the advanced beneficiary notices.
  • Participate with the interdisciplinary team in tracking and managing resident behavioral expressions and assisting with the development of person-centered plan of care to address these needs.
  • Make referrals and obtain needed services from outside entities (e.g., talking books, absentee ballots, community wheelchair transportation).
  • Provide referrals for residents (e.g., applying for pensions, referrals to advocates for creating advanced directives, referrals to funeral homes for preplanning arrangements).
  • Interactions with employees, residents, resident representatives, and guests will reflect our commitment to inclusivity.
  • Applies the concepts of the Mather Wellness Model and the 3 A’s in the care of residents, Achievement, Autonomy, and Affiliation.
  • Is competent in acting in emergency situations based on Safety Manual, Internal and External Emergency Preparedness and other policies.
  • Complies with Mather Code of Conduct, Policies, Compliance Programs, and all applicable laws and regulations in performance of one’s job duties; reports all known and suspected violations to his/her supervisor or manager, Mather Compliance Officer, a Human Resources representative, or the confidential employee helpline.
  • Identifies care center issues, proposes possible solutions and successfully adapts to changes as part of Quality Assurance Process Improvement process.
  • Documents care per documentation policy and standards.
  • Assists in licensing and survey preparedness and processes.
  • Operates efficiently within established parameters to maximize available resources.
  • Participates in sales and promotion of the community.
  • Incorporates fiscal responsibility through the provision of efficient, safe and cost-effective practices in the delivery of care.
  • Increases personal growth and development through formal and informal education programs.
  • Facilitates teamwork between functional departments to promote effective operations.
