About The Position

The Our House Residential Care Facility (RCF) is a 14-room facility with a team of expert clinicians who work with people living with HIV who are having difficulty managing independent living. By tailoring nursing, social services, and therapeutic everyday activities our clients and residents are able to define what ‘living well’ means to them. Our House of Portland a program apart of Cascade AIDS Project is excited to announce that we are hiring skilled and team-oriented Social Services Coordinator. The Social Service Coordinator’s role is to support Our House residents on their journey to mental and physical wellbeing. The Social Service Coordinator will work collaboratively with the RCF team to identify residents' needs and serve as a liaison between residents and community resources. This is a Full-Time role working at Our house of Portland located in SE Portland. The schedule for this role is Tuesday-Saturday 8:30-5:00pm. With an evening shift on Wednesdays with your choice of 10am-6pm or 11am-7pm Starting pay is $25.60 an hour which is equivalent to just over $53,248 a year. The Social Services Coordinator’s pay increases with tenure and the top of the pay scale is $29.45. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.

Requirements

  • GED or High school equivalent
  • Current Certified Recovery Mentor (CRM) or Peer Support Specialist (PSS) certification or able to obtain within 6 months of employment
  • Must be able to pass a DHS background check.
  • Current Unrestricted Oregon Driver’s License
  • Ability to be insured under employers’ auto insurance.
  • Strong communication skills, warmth, and humor.
  • Willingness to work with people with physical and emotional health challenges.

Responsibilities

  • Assist individuals in developing their own plan for defining and advancing their path to overall wellness with person centered care, harm reduction techniques, motivational interviewing and therapeutic boundaries.
  • Works in collaboration with Deputy Director of Social Services, Nursing staff, Caregiving staff, Resident Engagement and community partners to provide wrap around services.
  • Transport residents with company vehicles for routine trips (DMV, SSA, bank) and when other needs are identified by management to doctor appointments, mental health, grocery shopping as applicable to their goals.
  • Identify barriers (internal and external) to full participation in community resources and develop strategies to overcome those barriers.
  • Leads group in house process meeting.
  • Meets with individual residents as needed to update recovery plans
