About The Position

The Lutheran Home Skilled Nursing Community is hiring a Social Services Coordinator to support resident well-being and coordinate essential psychosocial services within our skilled nursing setting. Requirements

Minimum of 2 years of experience in a Skilled Nursing Facility (SNF) in a Social Services or similar role
Required experience: Discharge planning in a SNF
Experience with customer service and resident/family support
Related experience and/or training; or equivalent combination of education and experience

Responsibilities

Coordinate outside psychiatric and psychosocial services to support resident care plans
Conduct social service assessments and interviews to identify resident needs
Provide counseling, advocacy, and discharge planning support
Connect residents and families with community resources
Encourage participation in individual and group programming
Facilitate family involvement in care planning and transitions
Assist residents with legal documentation and care coordination
Maintain accurate documentation of social service plans and progress notes
Collaborate with interdisciplinary teams to ensure comprehensive resident care
