About The Position

The Lutheran Home Skilled Nursing Community is hiring a Social Services Coordinator to support resident well-being and coordinate essential psychosocial services within our skilled nursing setting. Requirements

  • Minimum of 2 years of experience in a Skilled Nursing Facility (SNF) in a Social Services or similar role
  • Required experience: Discharge planning in a SNF
  • Experience with customer service and resident/family support
  • Related experience and/or training; or equivalent combination of education and experience

Responsibilities

  • Coordinate outside psychiatric and psychosocial services to support resident care plans
  • Conduct social service assessments and interviews to identify resident needs
  • Provide counseling, advocacy, and discharge planning support
  • Connect residents and families with community resources
  • Encourage participation in individual and group programming
  • Facilitate family involvement in care planning and transitions
  • Assist residents with legal documentation and care coordination
  • Maintain accurate documentation of social service plans and progress notes
  • Collaborate with interdisciplinary teams to ensure comprehensive resident care
