CHAT is accountable for implementing an institution-wide system for healthcare transition while patients are at CHLA and for transfer of care from CHLA to adult providers. This position will be within our Navigation Hub. The Navigation Hub is accountable for providing individualized case management support to referred patients and families. This case management is aimed at supporting the patient/family with transferring to adult providers, insurance navigation, conservatorship or healthcare power of attorney, and public benefits identification and applications. The Social Services Casework is under the supervision of a Master's level social worker, provides specific services to patients and their families. Coordinates case management services. Demonstrates commitment to the principles of family-centered care.
