About The Position

Tabor Hills Senior Living Campus, a not-for-profit community located in Naperville, IL, has been dedicated to caring for our senior population for over 130 years. Tabor Hills Senior Living offers an Independent Living Community, Supportive Living Community and a skilled nursing center. Tabor Hills has a 5 Star Quality Rating by CMS. We have a rare opportunity for a part time Social Service Associate to join our team at our skilled nursing center. The hours are Tuesdays, Wednesdays and Thursdays from 9:30 a.m. to 6:00 p.m. COMPETITIVE PAY BASED ON EXPERIENCE$19.00-$20.00 PER HOUR ILLINOIS PAID LEAVE FOR ALL

Requirements

  • Education: Bachelor’s degree in human services or related field.
  • Experience: One year’s experience with geriatric and/or long-term care facility preferred.
  • Physical Demands: Lifting 30 pounds maximum with frequent lifting and/or carrying of objects weighing up to 20 pounds.
  • Mobility: Essential. Standing, walking, stooping and ability to push wheelchairs.
  • Working Conditions: Inside a well-lighted, well-ventilated, highly visible area.
  • Principal Contacts: Involved with residents, personnel and visitors under all conditions and circumstances.

Responsibilities

  • Prepare Social Services Admission paperwork and complete with Resident/Families.
  • Under the supervision of Director of Social Services, will complete necessary assessments including MDS, cognitive assessments, documentation in progress notes, social history, and care plan.
  • Follow the outlined Care Plan approaches and document resident response on a quarterly basis, or sooner if requested.
  • Participate in training related to the Psychosocial program (i.e., consultation visits, seminars, workshops, reading assignments).
  • As resident’s needs are identified, referral to an appropriate group or other intervention will be made.
  • Addresses Resident and Family concerns and complaints with professional discretion.
  • Demonstrate flexibility and adaptability when called upon by supervisor to fulfill additional responsibilities that relate to the psychosocial well-being of the resident population.
  • Assists with facilitation of resident room moves by contacting families and talking to the resident.
  • Assists Admissions with tours and contracts as required.
  • All other duties as assigned by the Director.
