Requirements

Previous SSA or SNF experience required

Responsibilities

Function as patient advocate, helping ensure they have access to services, connecting them with community resources
Completing quarterly assessments for long term patients
Supporting discharge planning
Establishing firm boundaries with patients with goal of helping them achieve maximum autonomy and independence
