About The Position
We are seeking a qualified Social Worker to assist our patients in our Mount Pleasant/Downtown Charleston area. This candidate should be a compassionate Social Worker with excellent customer service skills to assist our patients' multiple facilities in the area. This role would be traveling from the Charleston area. This is a salary-based position, with the opportunity to receive a monthly bonus. This is a full-time 8-hr hybrid position (8:00AM-5:00PM) (Monday-Friday). This role would be working with patients to help complete their advance care planning, cognitive assessments, placement, general resources, etc.
Requirements

Responsibilities
- Initiating Advance Care Planning for patients throughout their assigned Care Team (including facilities).
- Completing Daily Comprehensive Assessments on patients.
- Completing Daily Care Plans on patients.
- Documenting Chronic Care Management time daily.
- Meeting Daily Productivity Goals
- Coordinating discharges at contracted facilities
- Coordinating Placement.
- Continuation of coverage (i.e. coordinating acute visits for falls, med changes, etc.)
- Build and maintain professional relationships with referral sources.
- Maintain strong communication and professionalism between referral sources, patients/families, and Care Teams.
