About The Position

The Skilled Service Coordinator is responsible for managing and facilitating the admission process for all new skilled residents, ensuring a seamless transition into the nursing home. This role serves as a key liaison between residents, families and staff to promote a comfortable, supportive and high-quality care experience. The coordinator proactively addresses concerns, resolves issues and ensures resident satisfaction throughout their stay. Skilled Service Coordinator Full-Time Shift: Monday- Friday 8:00am-5:00pm

Responsibilities

Admission Coordination
Oversee and coordinate all aspects of new resident admissions.
Ensure a smooth, welcoming and efficient onboarding experience.
Collaborate with clinical and administrative teams to prepare for arrivals.
Resident & Family Support
Serve as the primary point of contact for residents and their families.
Maintain resident communication to ensure needs, expectations and concerns are addressed
Provide guidance and support throughout the residents’ stay.
Care Experience Oversight
Monitor resident satisfaction and comfort levels
Conduct check-ins with residents and families to assess well-being and service quality.
Advocate for residents’ needs with interdisciplinary teams.
Interdepartmental Collaboration
Work closely with nursing, therapy, social services and administrative staff
Ensure continuity of care and effective communication across departments.
