About The Position

The Services Coordinator implements and oversees administrative procedures related to OCHN’s agreements with its network of service providers. The Services Coordinator works collaboratively with other members of the OCHN Provider Network Team, other OCHN teams, and the provider network to ensure appropriate adherence to administrative procedures. This activity supports effective implementation of OCHN’s strategic vision and plan with respective service network, including initiatives and projects that advance involvement of individuals served, quality outcomes, performance improvement, cost analysis, and contract compliance.

Requirements

  • Bachelor's degree in relevant discipline required.
  • Minimum of 2 years relevant experience in human services administration, contract management, or related area, preferably in the public sector.
  • Knowledge of public mental health service delivery systems in Oakland County.
  • Strong interpersonal and communication skills.
  • Be able to articulate a complex service system while establishing positive rapport with individuals in need.
  • Knowledge of OCHN provider network.
  • Demonstrated proficiency with Microsoft Office 365 and other relevant software programs.
  • Demonstrated high level of interpersonal skills both in direct personal interactions and on the telephone.
  • Demonstrated high level of verbal and written communication skills.
  • Demonstrated ability to handle a high volume of relatively complex assignments accurately and timely.
  • Demonstrated organizational skills needed to establish and follow office procedures and maintain files and records.
  • Understanding of Michigan Mental Health Code and recipient rights policies.
  • Must have available means of transportation to and from OCHN and for required offsite meetings or site visits.
  • Must be available for meetings and events which may occur outside of standard office hours.
  • Must be able to complete all physical requirements of the job with or without a reasonable accommodation.

Responsibilities

  • Implement and oversee administrative procedures for Oakland Community Health Network (OCHN) contracted providers in accessing services and supports for individuals served.
  • Act as primary liaison between OCHN contract providers to support adherence to administrative procedures and to resolve barriers to individuals served in accessing services.
  • Coordination with OCHN contracted providers to oversee availability of services and support within the OCHN network.
  • Ability to track data by creating spreadsheets, tracking MDHHS and OCHN data, monitoring data, and communicating out to the core provider networks.
  • Participate in workgroups and committees as assigned.
  • Must have knowledge of MDHH’s Waiver Services Application system in order to open new cases, review and approve annual recertifications for all State-run Waiver programs.
  • Must be proficient in OCHN’s EHR for insurance coverages.
  • Must be able to communicate and collaborate with internal departments and external providers in order to ensure continuum of care for individuals served.
  • Ability to work within our internal system to create and track long-term care agreements.
