About The Position

The Service Coordinator role involves assessing and verifying service coordination needs, developing service plans, making referrals, and monitoring the provision of services. Key responsibilities include advocating for consumers, providing after-hours crisis services, and meticulously documenting progress. This position requires frequent interaction with consumers, their family members, and various service providers. The successful candidate must be able to adapt to consumer and family needs, which necessitates a flexible work schedule, including availability during after-hours, holidays, and weekends.

Requirements

  • Bachelor's or advanced degree from an accredited college or university in a social, behavioral, human service, or health-related field including, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice
  • OR a High School diploma or GED, plus graduation from Partners in Policymaking and personal experience as an immediate family member of an individual with intellectual and developmental disabilities
  • Valid Texas driver's license and an acceptable driving record
  • Personal automobile liability insurance as required by the state of Texas

Nice To Haves

  • One or more years of work experience serving individuals with intellectual and developmental disabilities
  • Working computer skills
  • Bilingual in Spanish & English
