About The Position

As a Service Coordinator in our Assertive Community Treatment (ACT) team, you will play a vital role in supporting individuals with severe and persistent mental illnesses, primarily schizophrenia. Your expertise will help provide crisis support and intensive outpatient services that empower clients to maintain stability in their communities, particularly after discharge from psychiatric facilities. You will engage in comprehensive service coordination, performing assessments, treatment planning, and advocating for clients as they navigate their recovery journey.

Requirements

Bachelor's degree in Human Services, such as Social Work, Psychology, or Counseling.
One (1) year of experience in case management or service coordination services demonstrating a solid foundation in addressing the needs of individuals with severe and persistent mental illness.
Must be registered with the Board of Counseling to practice as a QMHP or register with the Board of Counseling to practice as a QMHP trainee within the first 30 days of employment.
The selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
