About The Position

Under general supervision, plans, organizes or manages the activities of social service or community programs. Oversees the program or organizations policies regarding participant involvement, requirements and benefits.

Requirements

  • Bachelors degree in a related field from an accredited college or university AND two years of professional experience in a human services field.

  • DBHDD/APS/Medicaid clinical policies, procedures and regulations preferred
  • Out-patient behavioral health clinic operations and procedures

Responsibilities

  • Creates and maintains a high performance environment characterized by positive leadership and a strong team orientation.
  • Accomplishes site human resource objectives by interviewing, selecting, orienting, training, assigning, scheduling, coaching, and counseling, employees; communicating job expectations; adhering to policies and procedures.
  • Motivates staff to improve quantity and quality of work performed and provides training and development opportunities as appropriate.
  • Complete client intakes based on interviews with clients, their families, significant others and appropriate community agencies according to Agency standard; From information gathered during intake/assessment, develop or complete MICPs or individual service plans and participate and provide relevant input in scheduled treatment team meetings.
  • Contacts other health care professionals within or outside the organization to obtain consumer background information in accordance with established guidelines.
  • Assist clients in locating and utilizing community resources including legal, medical, financial assistance, and other referral services; Develops and effectively maintains a network of appropriate community contacts; Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on client's overall progress.
  • Transports or makes arrangements for transportation of, clients to appointments in a timely manner; Schedules follow-up appointments in a timely manner to ensure services are provided; Meet with client's continuity of care representative as required.
  • Facilitate/ lead groups on daily skill building-life skills, behavior management, and youth services and/or oversee the development of group curricula
  • Provides appropriate input in strategy or review sessions, committees and task forces to facilitate improve case management, collaboration, linkages and communication.
  • Advocate (if necessary) and conducts individual sessions in order to provide appropriate guidance and direction pertaining to personal and social development according to establish guidelines and standards.
  • Recognizes the need for client crisis intervention and reacts appropriately to routine situations and/or promptly notifies person on call or immediate supervisor of any crises, incidents or unusual situations.
  • Create, complete and sign progress notes/documentation every session/interaction collaboratively with clients according to Agency standards
  • Attends all required training, supervision, and staff meetings within the Agency.
  • May be shadowed by peers on the job
  • Perform other departmental duties as assigned or as needed in order to not interrupt services
