CB PACE Senior Outreach Specialist

This dedicated and dynamic Senior Outreach Specialist role is responsible for driving community engagement and referral growth across Alexandria, Fairfax, and Arlington. The specialist will build strategic relationships, increase awareness of the PACE model of care, and guide prospective participants through the enrollment process. This role serves as a key ambassador in the community, working closely with healthcare providers, senior living communities, and local organizations to expand access to high-quality, coordinated care for older adults.
Requirements
- Bachelor's degree in Marketing, Healthcare Administration, Social Work, or related field (or equivalent experience)
- Minimum of 5+ years of experience in outreach, business development, healthcare sales, or community relations—preferably within senior healthcare, hospice, home health, or PACE
- Proven success in developing and maintaining referral relationships with healthcare providers and community organizations
- Strong working knowledge of Medicaid, Medicare, and community-based senior services
- Valid driver’s license and willingness to travel locally for outreach activities
- Proof of current auto liability insurance
