About The Position

The Linkage Specialist will coordinate bi-directional referrals and hand-offs between HIV and HCV Prevention and Care and Substance Use Disorders/Co-Occurring Disorders (SUD/COD) interventions and treatment, providing Care Coordination, Navigation, Benefits Navigation, and linkage to other Supportive Services that further the goals of the SAMHSA-funded IMPACT program. Provide non-medical support services including housing, mental health and behavioral health services, nutrition, and any other services that address barriers to care and healthy living. Coordinate referrals of clients who screen positive for SUD/COD and link and navigate clients to IMPACT program. Connect individuals with hepatitis C to treatment and support services. Screen individuals for Pre-Exposure Prophylaxis (PrEP) and connect them with the PrEP program. Conduct intake assessment of needs appropriate to patient's age, health conditions, and psychosocial factors and complete support service referrals pertinent to identified needs. Develop a service plan of action for meeting client’s assessed needs. Conduct assessment of insurance status and provide benefits navigation to meet client’s individual service needs, including with specific patient assistance programs such as ADAP, PrEP and PEP, and HCV treatment. Conduct Rapid HIV testing for high-risk individuals and refer to prevention services. Maintain accurate, up-to-date and complete client charts, utilize program-specific documentation templates, and enter client data in a timely fashion in program-related database. Utilize program-specific evidence-based interventions and other quality service delivery strategies to implement appropriate client-centered counseling and education. Demonstrate effective communication skills necessary to interpret age-appropriate response to client service/treatment plan. Participate in case conferences and regular supervision with immediate supervisor and other regular staff and program team meetings as required. Attends necessary training and in-services. and regular supervision with immediate supervisor as scheduled. Complete in timely fashion other job-related duties to meet program needs as assigned by the Program Manager Perform other duties as assigned.

Requirements

  • This position requires a minimum formal education a bachelor’s degree in human services fields such as Social Work, Psychology, Public Health, or equivalent field and minimum 4 years of job-related experience. Although a bachelor’s degree is required, an associate’s degree with a minimum of 6 years of continuous job-related experience can be .
  • Master’s degree is highly preferred with minimum 2 years of job-related experience.
  • Required job-experience criteria include 2+ years’ experience in Case Management with expertise in substance use and mental health, minimum 2 years’ experience leading with grant-based funded project coordination including data tracking and reporting, experience using electronic medical records and electronic data tracking databases.

Nice To Haves

  • 2-5 years’ experience working with HIV or HCV treatment, care, and/or prevention
  • knowledge of HIV and sexual health prevention and treatment strategies and terminology
  • knowledge and experience with service delivery to special populations such as LGBTQ and communities affected by substance abuse and mental health
  • Intermediate computer experience, which includes but is not limited to proficiency in Microsoft Office Suite
  • Highly organized and self-motivated
  • Fluency in Spanish required
