About The Position

Under the supervision of the Community Based Services Administrator, manages the daily operations of the Senior Choice at Home program in conformance with policies, procedures, standards, and federal & state regulations. Responsibilities include oversight of the Senior Choice at Home Care program implementation, supervision of the Care Coordinators, and implementation for all sales and outreach activities. Additionally, this position acts as a liaison with the local businesses, healthcare community, and potential clients for the purpose of developing and maintaining new members and developing referral sources to achieve census goals.

Requirements

  • Bachelor’s degree or equivalent education and experience in related field preferred.
  • Prior experience related to care coordination preferred.
  • Prior experience preferred in sales and marketing in a healthcare environment.
  • Excellent oral and written communication and presentation skills.
  • Excellent interpersonal and leadership skills to effectively communicate and train on the systems and processes.
  • Must be self-starter, attentive to detail and possess excellent organizational skills.
  • Ability to manage and stay motivated.
  • Must be able to work with little supervision.
  • Positive attitude and approach to change and improvements.
  • Proficient in Microsoft Office (Word, Excel and PowerPoint).
  • Ability to quickly learn and utilize CRM software.
  • Effective sales, leadership and presentation skill, including group presentations.
  • Detail oriented and excellent organizational and follow-through skills.
  • Ability to problem solve in an effective manner to maximize referrals from sources.
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.

Responsibilities

  • Provides oversight of the membership enrollment process to ensure a consistent approach is used when screening individuals to qualify for the program.
  • Supervises the Care Coordination schedule to ensure health check-ins are scheduled consistently, efficiently, and timely.
  • Builds relationships with members through a variety of activities to ensure retention.
  • Reports on census and other key metrics monthly to VOANS.
  • Directs, supervises, and evaluates the performance of the Care Coordinators.
  • In consultation with the Community Based Services Administrator, develops and implements policies, procedures and standards for the program. Communicates changes to staff, monitors outcomes, and makes changes as needed.
  • Interviews and hires staff reporting to the Senior Choice at Home Manager.
  • Provides for orientation, ongoing training and evaluation of staff according to program needs, and state and federal regulatory requirements.
  • Fosters team-building, professional relationships and good communication among staff and other departments. Routinely communicates and implements changes in program goals, philosophy and policy to staff.
  • Identifies and markets to potential members for Senior Choice at Home program
  • Identifies professional referral sources and key decision makers in the primary and secondary markets
  • Pre-qualifies referral sources to determine their ability to send qualified leads to the community.
  • Visits and conducts presentations to area referral sources and key decision makers. Establishes and maintains professional relationships so that they will be comfortable referring their contacts to the community.
  • Develops a formal relationship-marketing plan in conjunction with the Community Based Services Administrator. The plan will define “priority accounts” in the primary and secondary market, establish referral goals, and determine the sales activity necessary to maintain these accounts.
  • Maintains accurate, complete, and up-to-date Customer Relationship Management (CRM) database of professional contacts to measure community relations’ efforts.
  • Keeps updated on relevant operational and competitive information in order to respond effectively to service inquiries from area professionals and ensure proper positioning of the community in the marketplace.
  • Participates regularly in meetings, training and seminars.
  • Identifies and coordinates on-site business meetings; and other marketing-related events as required.
  • Participates in local associations and groups with memberships and agendas that impact area seniors.
  • Assists in special event planning.
  • Other duties as assigned by Supervisor.
