About The Position

The Senior Change Management Practitioner will lead strategic change initiatives across our healthcare organization. This role is pivotal in driving adoption, engagement, and sustainability of enterprise-wide transformation efforts, including digital health, clinical optimization, operational excellence and patient experience. The Senior Change Management Practitioner will have healthcare industry knowledge, strong leadership capabilities, and a proven track record of managing complex change in matrixed environments.

Requirements

  • Bachelor's degree in Healthcare Administration, Organizational Development, Business, or related field
  • Prosci or equivalent change management certification required.
  • Minimum seven (7) years experience in change management, with at least three (3) years in a senior or lead role within a hospital or health system.

Responsibilities

  • Lead the development and execution of change management strategies for system-wide initiatives, including clinical transformation, digital health, and operational excellence.
  • Partner with hospital executives, clinical leaders, and administrative teams to ensure alignment of change efforts with strategic priorities and patient care goals.
  • Conduct impact assessments, stakeholder analyses, change risk evaluations and readiness evaluations across hospitals and service lines to inform change plans.
  • Design and implement communication, engagement, and training plans tailored to diverse healthcare audiences, including physicians, nurses, and support staff.
  • Champion a culture of change agility and continuous improvement across clinical and administrative functions by facilitating change leadership workshops and coaching sessions to build internal capability and resilience.
  • Monitor and report on change effectiveness, adoption metrics, and business outcomes. Incorporate feedback and outcomes to continuously improve change strategies and ensure sustainability.
  • Collaborate with Portfolio Management, HR, IT, and clinical operations to embed change management into project lifecycles and governance structures.
  • Serve as a mentor to junior change practitioners and project teams across the healthcare system on best practices and frameworks.
