About The Position

The Senior Case Manager plays an essential leadership role within the organization, providing comprehensive case management services to men and women impacted by the justice system. This position supports clients through individualized service planning, resource coordination, and consistent advocacy to address barriers related to housing, employment, behavioral health, and community reintegration. The Senior Case Manager also serves as a mentor to other case managers, ensures compliance with program standards and documentation requirements, and collaborates with community partners to strengthen service delivery. Through trauma-informed, client-centered practices, the role directly contributes to reducing recidivism and promoting long-term stability and successful reentry outcomes.

Requirements

  • Bachelor’s degree in criminal justice or social sciences OR four years’ experience with case management and/or group facilitation in the topical areas of criminal justice or social s
  • Certified Community Health Worker or the ability to obtain certification within 90 days of hire.
  • Valid California Driver’s License and auto insurance.
  • Ability to pass background check.
  • Treat staff, colleagues, and clients with respect and dignity.
  • Communicate effectively, written and orally.
  • Physical capacity to lift and carry containers and materials up to 20 pounds (utilizing safe and accepted methods to avoid injury), walk .25 miles in inclement weather and in the dark, and move quickly, in case of an emergency.

Responsibilities

  • Complex Case Management: Handle more challenging cases requiring significant independent judgment.
  • Indirect Supervision & Training: Provide guidance, training, and functional supervision to junior case managers.
  • Team Lead.
  • Sets the pace for productivity and quality service.
  • Program Oversight: Oversee intake, case management systems, and ensure compliance with regulations.
  • Complete case audits of other case managers for quality assurance.
  • Advanced Support: Offer counseling, crisis intervention, and support with life matters, leveraging advanced knowledge.
  • System Navigation: Guide clients and families through complex systems, such as conditions of parole, credit repair, public benefits
  • Process compliance referrals from probation.
  • Initiate communication and services with referred clients.
  • Assess and assist with addressing client challenges and barriers to maintaining compliance with probation requirements.
  • Assess client strengths and needs in areas such as employment, medical/mental health needs, housing, transportation, legal services, education, etc.
  • Develop case management plans and ensure ongoing monitoring and evaluation of progress and effectiveness of case plans.
  • Conduct home visits, meet clients off-site when necessary, and assist clients with resource navigation.
  • Attend all required meetings, trainings, and agency events.
  • Maintain compliance with data entry, record keeping, and documentation within the electronic database and client files.
  • Complete all reports as required.
  • Provide services in a manner that is trauma-formed, client centered, and culturally competent.
  • Lead team meetings in the absence of the Director of Reentry Programs.
  • Lead program data collection, reporting, and quality assurance for monthly and quarterly reports.
  • Other duties and tasks as assigned.
