About The Position

The Senior Housing Case Manager provides leadership and advanced support to agency’s housing programs by assisting with data management, training new staff, facilitating team meetings, and ensuring documentation and compliance standards are met. The role supports daily program operations, monitors staff schedules and performance, helps resolve issues, and collaborates with housing leadership to maintain consistent, high-quality service delivery. In this entry level supervisor role, the Senior Housing Case Manager maintains a caseload, meeting regularly with participants to develop individualized housing plans, coordinate services, link to community supports, and ensure accurate electronic record documentation. The position also provides coverage during staff absences, represents THRIVE in the community, and reports key concerns to program leadership. With assistance from the Assistant Director of Housing supervises assigned staff. Assists with carrying out supervisory responsibilities in accordance with the agency’s policies and applicable laws. Responsibilities include but are not limited to assisting in interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

Requirements

  • Bachelor’s degree in any of the following: child and family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation, recreational therapy, rehabilitation, social work, sociology, or speech and hearing AND two years’ experience either; providing direct services to persons with serious mental illness, developmental disabilities, alcohol or substance abuse, co-occurring disorders, geriatric population; OR linking persons who have serious mental illness, developmental disabilities, alcohol or substance abuse to a broad range of services essential to successfully living in a community setting
  • NYS licensure and current registration as an RN with a bachelor’s degree AND two years’ of the above experience
  • A credentialed Alcoholism and Substance Abuse Counselor (CASAC) and two years’ of the above experience
  • Bachelor’s level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses
  • Ability to perform each essential duty satisfactorily
  • Knowledge, skill, and/or ability required for the role

Responsibilities

  • Assist the Assistant Director of Housing and the Community Services Quality Director with collecting and maintaining data in evolve, CAIRS, HMIS and other reporting systems, as assigned
  • Responsible to train new housing case managers (CMs)
  • Facilitate team meetings to support staff and provide training in areas of concerns noted from internal and external audits and spot checks
  • Work within program budgets
  • Handle day-to-day questions and issues as they arise, adhering to agency policies and procedures and policies of the oversight agency
  • Assist housing staff by reviewing, evaluating, and further development of the individualized housing support plans
  • Assist Assistant Director of Housing and the Community Services Manager with day to day work within the program by supporting the team with completing assigned tasks and responsibilities
  • Oversee schedules, timecards and assists with completing annual reviews for assigned staff
  • Work closely with the Assistant Director of Housing and the Community Services Manager to problem solve and be proactive in staying on top of housing policies, procedures and documentation
  • Cover for absences and vacancies of CMs to provide consistency and continuity of care for clients
  • Provide guidance to other case managers, as needed
  • Represent the agency at community meetings, as assigned
  • Collect data for statistical purposes
  • Report to the Assistant Director of Housing all pertinent information concerning illnesses, accidents, untoward events, staff difficulties, repair needs, etc.
  • Meet monthly with each program participant on caseload to provide housing case coordination services
  • Develop written housing support plans based on strengths and individualized needs with the individual utilizing the “Golden Thread” methodology
  • Link recipients to all services and supports necessary to avoid homelessness
  • Link and refers program participants to needed rehabilitation, behavioral health, primary care, social and employment supports
  • Coordinate treatment with providers, as necessary
  • Perform problem-solving functions to assist individuals in overcoming obstacles to maintaining stable and secure housing and address other Social Determinants of Health needs
  • Maintain electronic health record documentation according to requirements of the THRIVE policies and procedures
  • Monitor and maintain high standard of service delivery
  • Maintain client confidentiality at all times
  • Follow agency policies and procedures in all program areas
  • Other job-related duties as assigned.
