Shift Available: Per Diem About DDI: For nearly 65 years, DDI (Developmental Disabilities Institute) has been a leading agency providing a full spectrum of care for over 1,600 children and adults with autism and other developmental disabilities. We are driven by a dedicated team and a deep belief in the potential of every individual. The Role: Provides one-on-one assistance to the individual in the home and community to enhance a variety of life skills to include social, recreational and activities of daily living. Assists the individual in identifying social, recreation and other community resources. Submits time sheets, mileage sheets, monthly expense reports, and service notes for services provided. Transports the individual to and from community activities. Teaches the individual all aspects of daily skills that pertain to the individuals valued outcomes in the community habilitation plan. Completes all training, in-service and re certification training as required. What You'll Do: Provide families temporary relief from responsibilities of daily caregiving. Ensure safety and well-being of the individual. Provide communication between families/individuals to ensure quality of services and follow up on any issues or concerns that may arise. Be familiar with fire and emergency procedures and provide support to the individual according to their safeguards. Complete and submit service documentation/timesheets and attend annual agency trainings. What You Need for the Role : Ability to read, understand, and follow safety guidelines. Good organizational, verbal, and written communication and interpersonal skills. Ability to navigate computers and programs. Working knowledge of OPWDD services and supports. Valid NYS Driver’s License. What You Must Be Able to Do : Must be able to modify the area to secure the safety of the individuals, e.g., move/push tables and other heavy objects up to 20 lbs. Must be able to lift/move /carry 20 lbs. Must be able to run after an individual, up to 500 feet. Must be able to run to a program in need , up to 500 feet. Must be able to kneel, twist , and bend.
