About The Position

In collaboration with the Senior Director of Resource and Referral and a contracted Social Care Network, the Specialist works to connect individuals and families with essential community resources. The Screening Specialist utilizes the Accountable Health Communities Health-Related Social Needs (AHC HRSN) Screening Tool to collect member consent, demographic information, and contact details. Screenings are conducted through a designated platform, either by phone or in person, with an emphasis on proper documentation, avoiding duplicate screenings, and ensuring eligibility for reimbursement. Working with the Senior Director of Resource and Referral the Screening Specialist will determine screening priorities and identify barriers to care and coordinating access to supportive services. The Screening Specialist helps promote the overall well-being and stability of Medicaid members.

Requirements

  • High School Diploma or equivalent required
  • Proficient computer skills
  • Demonstrated ability to work in a team environment with flexibility, keeping others informed and anticipating needs is essential
  • Must be efficient, well organized, and have excellent communication and interpersonal skills
  • Must be at least 18 years of age or older
  • No convictions for crimes that preclude a person from working with members of a vulnerable population

Nice To Haves

  • Associate’s degree preferred
  • Experience working with the Medicaid population preferred
  • Valid Driver’s License and satisfy criteria to be a driver for FREE – preferred

Responsibilities

  • Conducts screenings with Medicaid members utilizing a designated screening tool as per the respective Social Care Network.
  • Utilizes the Accountable Health Communities Health-Related Social Needs (AHC HRSN) Screening Tool to collect member consent, demographic information, and contact details.
  • Identifies and prioritizes Medicaid members to connect with to conduct screenings.
  • Accurately documents service provision, daily notes, and other information as required into the respective electronic health record.
  • Advocates with and for the people we support in a variety of situations.
  • Assist with connecting individuals and families with essential community resources, including but not limited to, housing, healthy food, transportation, and personal safety services.
  • Communicates clearly and effectively with others via phone call, email, and in person.
  • Ensures that all required documentation (paper and electronic) is completed/processed appropriately, in a timely manner, and meets quality and compliance thresholds.
  • Performs other duties as assigned.
  • Always communicates in a respectful manner with all people including the people we support, supervisors, other valued team members, and members of the community.
  • Complies fully with FREE’s confidentiality policies by holding in strictest confidence all information pertaining to members, residents, the people we support and their families, clients, providers, contractors, and employees.
  • Meets all safety training requirements; adheres to all safety policies and reports any and all unsafe conditions; takes measures to prevent accidents and/or eliminate conditions that pose a safety or health risk.
  • Coordinates, supports, attends and participates in all required events/ meetings/ reviews/ committees, including ones outside of standard working hours.
  • Meets all job requirements and stays in compliance.
  • Adheres to FREE’s standard of ethical behavior as outlined in the Employee Handbook and Code of Conduct as well as applicable laws and regulations.
