Access Community Care is seeking a detail-oriented and motivated individual to join our team as a Scheduling and Outreach Specialist (ADL & DSA Support). In this vital role, you will support the coordination of services, scheduling operations, and outreach efforts that ensure individuals with Intellectual and Developmental Disabilities (IDD) receive consistent, high-quality, person-centered support. In this position, you will play a key role in maintaining efficient schedules, supporting program coordination, and fostering strong communication between team members, customers, and community partners. This role works closely with Department Leadership to ensure adequate staffing, smooth program operations, and high-quality, person-centered services across both departments. Outcome: The primary objective of this position is to ensure efficient scheduling, strong communication, and effective program coordination that supports both ADL and DSA services. This role helps maintain continuity of care, promotes community engagement opportunities, and supports team members in delivering high-quality services.

Requirements

  • College degree preferred or equivalent experience
  • Must be 18 years of age or older
  • Clear background checks as defined by state regulations and company policies
  • Minimum 1 year of experience in developmental disability services or similar field
  • Minimum 1 year of scheduling, coordination, or lead experience preferred
  • Valid driver’s license, reliable vehicle, and current auto insurance required
  • Strong organizational, problem-solving, and critical thinking skills
  • Excellent verbal and written communication skills
  • Ability to manage multiple priorities and adapt in a fast-paced environment
  • Proficiency in Microsoft Office, Google Workspace, and electronic record systems
  • Understanding of person-centered practices and service documentation requirements
  • Ability to work collaboratively within a diverse team environment
  • Ability to perform physical tasks as needed (lifting up to 20 lbs., standing, walking, etc.)

Responsibilities

  • Manage and maintain daily and ongoing schedules for ADL staff to ensure full coverage
  • Monitor time and attendance, making real-time adjustments as needed
  • Review participant authorized hours and ensure compliance with service allocations
  • Serve as a primary point of contact for scheduling needs, call-outs, and coverage gaps
  • Maintain accurate scheduling records and tracking systems
  • Serve as a key point of contact for team members, participants, and case managers
  • Support outreach efforts including coordinating referrals, meet-and-greets, and onboarding processes
  • Facilitate clear and consistent communication across departments to support service delivery
  • Build and maintain positive relationships with internal teams and external partners
  • Assist with scheduling and coordination of DSA events, activities, and staffing needs
  • Support transportation and logistics planning for community-based programs
  • Collaborate with DSA staff to ensure events are adequately staffed and aligned with participant goals
  • Track participation, attendance, and program needs for continuous improvement
  • Track and follow up on incident reports within required timelines
  • Assist with documentation processes including intake forms, service agreements, and reports
  • Support Employee Performance Report (EPR) documentation and tracking
  • Maintain compliance with state, federal, and organizational requirements
  • Complete required trainings and support training of team members as needed
  • Provide direct support to individuals when necessary to maintain service continuity
  • Support individuals in community integration, socialization, and achieving personal goals
  • Ensure all services are delivered in a person-centered, respectful, and professional manner
  • Participate in rotating on-call responsibilities as assigned
  • Maintain flexibility to support both ADL and DSA operational needs
  • Demonstrate professionalism, accountability, and a team-oriented approach
  • Perform other duties as assigned
