Our mission is to provide each client and family with a sense of comfort, knowing they are being cared for by professionals that are exceptional in their field and who are innately invested in the well-being of others Job Summary: A Home Health Physician Orders Support Coordinator (POSC) primarily handles the efficient processing and management of physician orders for home health services, ensuring timely compliance and accuracy. This role involves coordinating with physicians to obtain signed orders, tracking their status, and communicating updates to relevant parties, including intake and Director of Nursing/ DON. A Physician Orders Support Coordinator is also responsible to submit orders via fax or doctor’s alliance platform to PCP’s office, obtaining signed orders for a new and current patient Brockton HHA patients, upload the orders into Axxess and tracking the order's status. POSC also need to contact the physician’s office to follow up if orders are not signed promptly. The POSC may need to call hospitals, or PCP’s office to request last visits notes or updated medication list after a hospital discharge before the agency can resume services.
