About The Position

The SAMHSA Case Manager (APPAL Program) is responsible for coordinating and monitoring all aspects of client engagement under the APPAL initiative. This role ensures the timely completion of required surveys, maintains accurate program tracking systems, and delivers high-quality case management services. The Case Manager collaborates closely with clinical, data, and compliance teams to meet SAMHSA grant requirements, improve client outcomes, and uphold agency-wide quality standards.

Requirements

  • Strong organizational and analytical skills with the ability to manage multiple priorities
  • Proficiency in Microsoft Excel, Google Sheets, and data visualization tools
  • Experience with clinical documentation and productivity tracking
  • Ability to develop and maintain tracking systems for deadlines and compliance
  • Strong reporting skills with the ability to identify trends and improvement areas
  • Excellent communication and collaboration skills across multidisciplinary teams
  • Bachelor’s degree in social work, Public Health, Psychology, or related field (master’s preferred)
  • Minimum of 2 years of case management or behavioral health experience
  • Proficiency in Microsoft Office Suite and EMR/case management systems
  • Strong organizational and time-management skills
  • Ability to work independently and collaboratively
  • Excellent written and verbal communication skills
  • Valid driver’s license and reliable transportation required

Responsibilities

  • Conduct new intake, follow-up, and exit APPAL surveys for assigned clients
  • Provide high-quality, client-centered support during all follow-up interactions
  • Ensure a minimum of three (3) clinical encounters per client prior to discharge
  • Travel to designated APPAL sites and maintain consistent on-site presence
  • Monitor active clients to ensure completion of required services
  • Maintain and update the APPAL master productivity tracker
  • Provide regular updates to the clinical team on client progress and follow-up needs
  • Ensure documentation is accurate, timely, and properly routed through workflow systems
  • Track all required clinical services and documentation per SAMHSA grant guidelines
  • Maintain knowledge of grant benchmarks and ensure compliance
  • Develop and manage tracking systems for grant deliverables
  • Ensure all program data and documentation are complete, accurate, and submitted on time
  • Partner with Data Manager to align tracking tools with reporting systems
  • Stay current on SAMHSA reporting requirements and compliance standards
  • Assist in resolving discrepancies between service delivery and reported data
  • Support audits, site visits, and grant reporting processes
  • Participate in clinical and administrative meetings
  • Provide data-informed recommendations to improve program performance
  • Identify workflow or service delivery challenges and escalate to leadership
  • Ensure all clinical encounters are accurately documented in required systems
  • Track progress toward quarterly and annual SAMHSA benchmarks
  • Generate monthly and quarterly reports on service completion and outcomes
  • Create summaries and data visuals for leadership review
  • Maintain strict confidentiality and adhere to HIPAA regulations
  • Participate in ongoing training (trauma-informed care, harm reduction, cultural competency)
  • Provide client advocacy and referrals to supportive services
  • Assist with crisis intervention and safety planning as needed
  • Serve as a liaison between clients, agency leadership, and external partners
