About The Position

Through a partnership with OHSU, the SAMHSA Hep C Elimination Coordinator works to expand peer-assisted telemedicine services across Oregon. The coordinator role will play a vital role in providing same-day HCV diagnosis and treatment in a truly innovative model of care. The Coordinator provides outreach, peer support, and transportation to Hep C testing sites, assists participants in connecting to telehealth appointments for diagnosis and treatment, and supports individuals in navigating follow-up care and community resources. By drawing on lived experience and peer engagement strategies, the Coordinator helps reduce stigma, increase trust in healthcare systems, and support individuals in taking next steps toward improved health and recovery.

Requirements

  • Identifies as having at least 2 years of lived experience in recovery with addiction challenges (may include both addiction and mental health challenges)
  • Associate’s degree and 1-2 years of relevant work experience OR a high school diploma and 3-4 years of relevant work experience
  • Understands the principles of trauma-informed care, recovery, consumer-involvement, and trauma-informed care
  • Peer Support Specialist (OHA Traditional Health Worker certification) or MHACBO CRM certification required or able to be obtained within 90 days of hire
  • Valid Oregon Driver’s License and proof of automobile insurance required. Access to reliable personal vehicle required
  • Ability to network and collaborate with diverse groups of people who have varied skills, knowledge, and goals
  • Strong written and verbal communication skills
  • Ability to work independently as well as collaboratively within a team
  • Ability to read, analyze, and interpret mental health and recovery periodicals, professional journals, and government regulations
  • Well-developed writing skills including the ability to write for publication (e.g., reports, technical assistance guides, and educational and training materials)
  • Typing and general technology skills to navigate electronic systems easily and efficiently (e.g., email, Microsoft Teams, data systems, etc.)

  • Experience with mental health recovery is desirable and highly valued

Responsibilities

  • Use a specialty point of care testing device that yields a confirmed HCV diagnosis within 60 minutes without labs
  • Use a specialty imaging device that determines liver fibrosis within minutes without any labs required
  • Complete dried blood spot testing for other infectious diseases
  • Coordinate with the peers and OHSU care manager to facilitate rapid linkage to screening and care for mental health disorders and linkage to telemedicine treatment for HCV and opioid use disorder
  • Coordinate with peers to reach participants for ongoing care, and assist with contacting participants for follow-up testing
  • Provide infectious disease prevention education to people who would like help with this
  • Enter relevant data into an online peer services database
  • Attend regular meetings with the OHSU implementation team
  • Complete all relevant training assigned by The Peer Company and OHSU to successfully utilize all systems, tools, and processes
  • Support team communication by preparing agendas, taking meeting notes, maintaining program files, and coordinating internal information flow
  • Respond to general inquiries about the SAMHSA Hep C Elimination program and redirect as appropriate
  • Collaborate across interdisciplinary teams to ensure smooth office and facility operations
  • Communicate with staff and supervisors in a timely, organized manner
  • Travels to visit community partners and participants. Regularly transports participants to Hepatitis C testing sites, healthcare providers, and related services
  • Perform other administrative and program support duties as assigned
