About The Position

RN System Director of Case Management Gainesville, Georgia The Director of Case Management leads and shapes the Case Management and Social Work services. As the Director of Case Management, you’ll guide discharge planning, Care Coordination, and access management to ensure patients receive seamless support throughout the Continuum of Care. Additionally, the Director oversees pre-certification for post-acute services, optimize resource use, and promote ICD-10 awareness. As a strategic leader passionate about improving patient transitions and building strong care connections, this role offers the chance to make a meaningful impact system-wide.

Requirements

• Registered Nurse
• Bachelor’s Degree required
• Director of Case Management experience in an Acute Hospital Setting required

Nice To Haves

• Master Degree preferred
• ACM or CCM preferred

Responsibilities

• Formulate and implement operational strategies and initiatives in the Case Management program
• Direct and evaluate all services provided, including care plan development, service level determination, and complaint management to achieve performance and quality control objectives
• Provide specialized expertise, ensure resource alignment with system-wide initiatives, and update appropriate parties on Case Management strategies, initiatives, outcomes and issues
• Successfully partner with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinical variation in care
