About The Position

RN Director of Case Management South Lake County, Florida The Director of Case Management has full accountability and oversight for the overall operations of the Care Management department. The Director of Case Management oversees patient Care Coordination to ensure high-quality, cost-effective outcomes. This role requires strong leadership to manage Case Managers and Social Workers, collaboration with physicians and interdisciplinary teams, and compliance with regulatory standards. The Director of Case Management must have a strong background in acute care hospital Case Management, along with demonstrated expertise in performance improvement and healthcare operations.

Requirements

• Registered Nurse
• Bachelors of Nursing required
• Director of Case Management experience in an Acute Hospital setting required

Nice To Haves

• MSN preferred
• ACM or CCM preferred

Responsibilities

• Directs the daily operations of the Care Management department
• Implements the Corporate Care Management strategies
• Assists in the development and oversees the implementation of Care Management policies, procedures, and standards
• Participates in the development of new staffing and care designs that support system goals while achieving best in class performance
• Supports standards that are consistent with hospital, community and nationally recognized evidence-based practice
• Works collaboratively with leaders of Utilization Management to quickly and efficiently address any Care Management or Utilization Management concerns or barriers
• Identifies process improvement opportunities and develops, implements and monitors action plans based upon identified Care Management metrics
