Full-time RN position available in our PACE Center, which provides integrated medical, rehabilitation, case management, and adult day health services all in one location. Responsible for day-to-day nursing assessment and for the delivery and documentation of quality patient care.

Requirements

  • Current MA RN License required
  • Current Massachusetts license in nursing.
  • Current BLS certification required.
  • Basic physical assessment and triage skills.
  • Ability to communicate assessment and triage findings efficiently, effectively and accurately in verbal and written formats.
  • Ability to communicate effectively with professional and non-professional staff, outside providers, participants and families in person and by phone.
  • Flexibility, initiative, creativity and excellent organizational skills.
  • Basic computer skills including experience with word processing and email.
  • Must be able to efficiently and effectively navigate windows-based programs and NeighborHealth’s EMR.

Nice To Haves

  • minimum of 1 year of experience in ambulatory, home health, hospital or sub-acute setting preferred.
  • A minimum of one year experience working with a frail or elderly population.
  • Prior experience in interdisciplinary team setting a plus.
  • Baccalaureate degree in nursing preferred.
  • Minimum of 2 years experience in ambulatory, home health, hospital or sub-acute setting preferred, equivalent training or related experience considered.
  • Bilingual a plus.
  • Excellent customer service skills.

Responsibilities

  • Assess a patient through obtaining history, performing appropriate tests, making or collecting observations from the participant and caregivers, and consulting with other members of the IDT.
  • Prepares and presents a patient for rapid provider review and treatment.
  • Arranges for or directly implements care plan interventions and testing, treatment, or other provider orders for a panel of participants.
  • Oversees delegated tasks to ensure completion, and monitor outcomes, and make adjustments to care plan as needed.
  • Safely and effectively administers medications, tests and treatments as ordered.
  • Manages the refills of all patient medications with appropriate authorization by the provider.
  • Teaches patients and families health care regimens.
  • Explains and provides information regarding disease and wellness, tests, results, medications, diet, exercise, and other aspects of the plan of care to patients.
