The Patient Safety Coordinator investigates significant safety and sentinel events and represents a key position in safety improvement and risk mitigation. Being a self-starter, having attention to detail, and having a passion for process improvement through causal analysis are key characteristics for this position. The Patient Safety Coordinator is responsible for supporting the hospital's Patient Safety program through focused reviews/investigations relative to patient/staff safety; facilitation of root cause analysis; presentation of reviews/investigations/root causes and associated data review and analysis. The Patient Safety Coordinator: Monitors enterprise-wide patient safety event reporting system (eSafe); Facilitates Centers for Medicare and Medicaid Services (CMS)-related sentinel event investigations; Facilitates apparent and root cause analyses of safety events, including interviews, HPI standard RCA meeting coordination, and document coordination; Aids in initial and ongoing risk and patient harm reduction processes; Facilities education of staff on ACA and RCA processes and development of corrective action plans; Communicates outcomes from ACA/RCAs to organization, as appropriate; Monitors action items from ACA/RCAs for completion; Helps coordinate data submission to relevant Patient Safety Organizations (Press Ganey PSO and Child's Health PSO); and, Additional responsibilities may include projects to identify and manage improvement opportunities for key operational and clinical initiatives.
