About The Position

The overall goal of this position is to navigate spine patients within the Texas Health Huguley system to the appropriate care destination and follow the patient throughout the care process. The Spine Nurse Navigator coordinates, delegates, provides, and evaluates care of the spine patients, including planning and delivering timely patient-focused care in partnership with interdisciplinary teams, the patient, and family. The Spine Nurse Navigator facilitates an evidence-based, multidisciplinary, multi-specialty approach to spine care throughout the care continuum at Texas Health Huguley. The Spine Nurse Navigator provides direct clinical service to patients and families including care management and patient education to understand diagnosis, treatment, and follow-up regimen. The patient needs may range from minimal to complex and may occur in the inpatient or ambulatory arena. The Spine Nurse Navigator is responsible for facilitating a patient and physician friendly experience within the Texas Health Huguley Spine Health Program. The Spine Nurse Navigator provide education and mentoring to advance staff knowledge at Texas Health Huguley, actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. The Spine Nurse Navigator assists in planning and conducting community outreach activities, including screenings, as well as assists patients in accessing appropriate resources as needed. The Spine Nurse Navigator facilitates patient support and/or education groups and strives continually for innovative and creative strategies to affect and improve patient quality of life. The Spine Nurse Navigator gathers abstracts, records and reviews data for process and quality improvement, as well as evaluating program goals.

Requirements

  • Bachelors in Nursing
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure in the state of Texas, or in a qualifying compact state
  • BLS Certification

Responsibilities

  • Coordinates patient care from entry point through discharge, ensuring timely and patient-focused care.
  • Reviews scheduled patients to ensure educational material, assessments, and pre-surgical care coordination follow timelines and protocols.
  • Develops and modifies individual care plans in collaboration with the spine program team.
  • Evaluates patients' developmental level, physical and emotional needs, and support system to enhance care and outcomes.
  • Completes follow-up assessments, investigates situations, evaluates interventions and medications, and modifies care plans as needed.
  • Provides age-appropriate care to spine patients.
  • Ensures patients' physical, mental, and spiritual needs are met by coordinating with the healthcare team.
  • Develops, coordinates, and documents patient education and care.
