Apply today! JOB SUMMARY: Responsibilities include coordination of patient care and management of surgical patients through their surgical journey. The Nurse Navigator will provide outstanding customer service and follow the patient throughout the pre-operative surgical phase through post discharge rehab and follow up care. Fosters an environment of teamwork and serves as a role model in the department. He/she will demonstrate actions and attitudes that contribute to the critical success factors of patient navigation in a community and inpatient setting. SCHEDULE: Monday - Friday 8:00 - 4:00 Weekend rotation every 6-8 weeks for 4 hours. No holidays.

Requirements

  • Licensed and currently registered as a Registered Nurse in the State of Pennsylvania
  • A minimum of 3 years of related
  • Strong clinical skills, specific to orthopaedics, as well as a well-rounded knowledge of the chronic care management of patients required.
  • Must be highly proficient in and knowledgeable of all areas of clinical paperwork and computer use, to include electronic medical records.
  • Must possess a high level of analytical skills and critical thinking skills necessary to evaluate problems and develop plans and procedures to achieve resolution, assure completion and evaluate effectiveness.
  • Independent judgment necessary to work in accordance with department practices and general directives from the VP Surgical Services
  • Ability to work independently and utilize time management, prioritization and multi-tasking skills.
  • Must be pro-active, flexible and able to work with different providers and staff to ensure patients’ needs are being met as well as staff satisfaction at an appropriate level.

Responsibilities

  • Assists in the identification of clinic patients eligible for
  • Assesses patient's ability to navigate through the healthcare system and identifies possible barriers to diagnosis and treatment.
  • Works with multidisciplinary team to maintain and implement evidence-based care pathway for total joint/spine patients.
  • Implements, evaluates, and revises surgical plan of care based on physical, psychosocial, spiritual, and educational needs of the patient and family or caregiver.
  • Ensures patient and caregiver are prepared for discharge (support system at home is adequate, medications prior to surgery are accurate, equipment needs are appropriately identified, follow up appointments have been scheduled and address any barriers prior to surgery).
  • Updates patient medical history into EMR along with medication
  • Assists patient in scheduling appointments for routine, diagnostic and/or follow-up care.
  • Assists patients in overcoming financial barriers to obtaining appropriate screening/diagnostic testing and treatment.
  • Monitors the receipt of testing and consult results and reviews abnormal results with anesthesia.
  • Follows-up with patient on results as directed by the
  • Documents significant patient navigation activities into the medical
  • Ensures coordination of care among treatment
  • Develops relationships with personnel and entities providing diagnostic and treatment services to the patient.
  • Tracks and trends clinic progress and effectively communicates findings to management and staff to promote screening services, risk stratification and follow-up to surgical patients.

  • Provides assistance to spread the learnings of patient navigation to other preventative and disease state patient navigation initiatives.
