Requirements
- Current RN license with valid credentials
- Prior home health nursing experience is required
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Reliable transportation to travel within Trenton and surrounding areas
- Ability to work independently and manage time effectively
Responsibilities
- Provide skilled nursing care to patients in their homes within Trenton and nearby communities
- Assess patient health status and implement individualized care plans
- Coordinate with physicians, therapists, and other healthcare providers to optimize patient outcomes
- Document all patient interactions, treatments, and responses accurately and timely
- Educate patients and families on health management and disease prevention
- Maintain compliance with all regulatory and company standards for home health services
