Requirements
- Valid Registered Nurse (RN) license in the state of Michigan.
- Prior experience in homecare nursing preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Reliable transportation to travel within Franklin and surrounding areas.
- Compassionate and patient-focused approach.
- Ability to work independently and manage time effectively.
Responsibilities
- Conduct comprehensive home assessments to evaluate patients’ health status and needs.
- Develop, implement, and monitor individualized care plans in collaboration with patients and healthcare teams.
- Provide skilled nursing services including medication administration, wound care, and disease management.
- Educate patients and their families on health maintenance, disease prevention, and treatment plans.
- Document all patient interactions and care provided accurately and timely.
- Coordinate with physicians and other healthcare professionals to ensure continuity of care.
- Maintain adherence to all regulatory and agency standards and policies.
