About The Position
We are seeking a Registered Nurse (RN) to join our Home Health team on a PRN (as needed) basis in Wayne County. The RN will provide compassionate and high-quality care to patients in the comfort of their homes, ensuring their well-being and promoting independence.
Requirements
- Valid RN license in the state of Michigan
- Minimum of 1 year of nursing experience, preferably in home health or a similar setting
- Strong clinical skills and the ability to assess, plan, implement, and evaluate patient care
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Reliable transportation and a valid driver's license
- CPR certification
Responsibilities
- Conduct home health assessments and develop personalized care plans for patients
- Administer medications, treatments, and procedures as prescribed by the physician
- Monitor patient progress and report any changes to the healthcare team
- Provide patient and family education on disease management and self-care techniques
- Coordinate with other healthcare professionals to ensure holistic care for patients
- Maintain accurate and timely documentation of patient care
