Your Role As a valued member of our interdisciplinary team, you’ll: Conduct in-home patient assessments Develop and implement individualized care plans Deliver skilled nursing interventions such as wound care and medication management Provide patient and family education on disease management and recovery Coordinate closely with physicians, therapists, and other care team members Document all care accurately and timely What We’re Looking For Current RN license (Washington State) BLS certification (AHA) Previous Home Health or Hospice experience preferred but not required Ability to work independently and use sound clinical judgment Reliable vehicle, valid driver’s license, and current auto insurance Join a supportive, mission-driven team that celebrates success, values collaboration, and believes in balancing professional excellence with personal well-being.
