Requirements

Graduate of an accredited nursing program and active RN license in Virginia
Minimum of (2) Two years of RN nursing experience, ideally in home healthcare
CPR certification
Knowledge of state and federal home health regulations
Clinical proficiency in nursing specialty, strong communication skills and compassion

Responsibilities

Develop and update individualized Plans of Care (POC) based on client needs, physician orders, and agency policies.
Ensure timely reassessments and supervisory visits per state and agency requirements.
Serve as a liaison among physicians, clients, families, and other interdisciplinary team members.
Participate in quality assurance activities, chart audits, and staff meetings.
Coach family members on the patient’s plan of care, medication and home safety
Communicate changes in condition, medication, or care needs promptly to the appropriate parties.
Maintain accurate and timely documentation in the electronic health record (EHR) system.
