About The Position
CHANS Home Health is searching for an RN Case Manager. The RN Case Manager provides high reliable, evidence-based, patient-family centered quality care for terminally ill patients in their homes. The Hospice RN Case Manager develops the plan of care in collaboration with patient and family goals. Our team is dedicated to our community, and we are passionate about what we do.
Requirements
- Graduate of an accredited School of Nursing required; BSN preferred.
- Current applicable state(s) license as a Registered Professional Nurse required.
- Current Driver's license with vehicle available for work covered by liability insurance as specified by agency policy.
- Current BLS Certification or must complete within 30 days of start date.
- 1+ years of previous experience required.
- Demonstrates competency in all essential functions of a Registered Nurse - Case Manager I role.
