About The Position
Registered Nurse Home Health Case Manager- St. Barnabas Home Health Registered Nurse – Home Health (Full Time)--New Hire Incentive of up to $3000 St. Barnabas Health System | Gibsonia, Beaver, and Surrounding Communities St. Barnabas Home Health is growing — and we’re looking for dedicated RNs to join our expanding team. Requirements
- Current RN license in Pennsylvania
- Prior home health experience preferred
- Strong knowledge of OASIS and CMS Guideline
- Reliable transportation and strong time management
Responsibilities
- Complete OASIS assessments (Start of Care, Recert, Resumption, Discharge)
- Provide skilled nursing services: wound care (including complex wounds), IV therapy, and medication management
- Support chronic disease management and patient/family education
- Work closely with PT/OT/ST and the full interdisciplinary team
- Follow CMS, Medicare, and PA DOH standards with confidence and accuracy
