Position Overview: We are seeking a dedicated and experienced Registered Nurse (RN) Case Manager to join our home health care team. As an RN Case Manager, you will be responsible for coordinating and providing comprehensive care to patients in their homes, ensuring they receive high-quality care tailored to their individual needs. Requirements

  • Current and unrestricted Registered Nurse (RN) license in CA
  • Minimum of 1 year of clinical nursing experience, with home health or related experience preferred.
  • Excellent clinical assessment and critical thinking skills.
  • Strong communication and interpersonal skills.
  • Proficiency in electronic health record (EHR) systems.
  • Valid driver's license and access to a reliable vehicle for travel to patient homes.
  • Cultural competence and sensitivity to diverse patient populations.

Responsibilities

  • Conduct initial assessments of patients' health conditions and create individualized care plans.
  • Regularly assess and reassess patients' needs and update care plans as necessary.
  • Provide skilled nursing care, administer medications, and perform treatments as prescribed.
  • Educate patients and their families on proper care techniques and medication management.
  • Coordinate with the interdisciplinary healthcare team, including physicians, therapists, and social workers, to ensure holistic patient care.
  • Monitor patients' progress, document all care activities, and maintain accurate patient records.
  • Evaluate the safety and cleanliness of patients' homes and make recommendations for necessary modifications.
  • Provide emotional support and counseling to patients and their families.
  • Ensure compliance with all applicable healthcare regulations and guidelines.
  • Participate in case conferences and team meetings to discuss patient care and coordinate services.
