About The Position

POSITION SUMMARY: Responsible for developing, implementing, and administering the community health center’s risk management program which is aimed at improving patient care, reducing the risk of adverse outcomes, and complying with regulatory and accreditation requirements.

Requirements

  • Bachelor's Degree in Healthcare or related field
  • 4 years of experience in healthcare directing and implementing performance/process improvements, risk management, patient safety
  • Must possess a valid Texas Driver’s License and be insurable
  • Must be able to successfully pass a post-offer drug screen and criminal history background check
  • Knowledge of policy development in a health care environment
  • Knowledge of how to conduct patient safety and staff awareness
  • Understanding of how to design, implement and maintain educational programs on risk management and patient safety topics
  • Knowledge of federal and state laws and regulations through policy development, guidance, and education
  • Knowledge of exposure control, including bloodborne pathogens and respiratory protocol protections
  • Considerable knowledge of health care law, safety, regulations, and standards
  • Ability to establish program goals and objectives and to assess progress toward their achievement
  • Proficient in MS Office applications and database software
  • Ability to effectively present information and respond to questions from management and the public
  • Excellent customer service attributes
  • Must be able to prioritize and manage multiple tasks effectively
  • Must have excellent organizational and time management skills
  • Knowledge of electronic medical records systems

  • Certified Professional in Healthcare Risk Management (CPHRM) a plus

Responsibilities

  • Conducts risk assessments, collecting and analyzing documentation, statistics, and reports
  • Responsible for coordinating activities resulting in accreditation
  • Conducts and facilitates risk management educational presentations
  • Abstracts charts for core measures, coordinates internal mock surveys, and serves as a resource in complying with the Accreditation body
  • Creates and maintains Risk Management Policies and Procedures and reviews other division's policies and procedures to ensure compliance with accreditations and patient safety standards
  • Oversees the compliance activities throughout the department by collecting, auditing, and analyzing clinical quality programs
  • Responsible for advising, reviewing, and assisting in implementing risk management plans
  • Responsible for identifying, evaluating, and reducing the risk of injury to patients, staff, and visitors and the risk of loss to the department
  • Collaborate with other divisions by preparing and conducting quality and/or risk assessments to maintain a constant state of accreditation readiness
  • Responsible for investigating and resolving patient/client complaints and concerns
  • Coordinate performance improvement activities
  • Serves as a member of the Performance Improvement Committee (PIC) and Safety Committee
  • Work closely with infection control personnel to mitigate risk for patients and employees
  • Coordinates with clinical staff on electronic health records data entry pertinent to patient services or interventions provided regarding risk management issues
  • Coaches physicians, leaders, managers, and staff on appropriate disclosure methods and processes
  • Assist in the investigation of sentinel events and/or near misses using root cause analysis
  • Manages incident reporting and data analysis for patient safety, patient complaints, and other known risk events
  • Educate providers, staff, employees, patients and families on the role of patients and families in improving patient safety and reducing risk
  • Interacts with legal counsel, consultants, and auditors as needed
  • Prepares and submits monthly reports
  • Performs other position-related duties as assigned and required.
